Case Log Submission – UNDER CONSTRUCTION – DO NOT USE
The resident case log is to be submitted as an annual report of the anesthesia cases for which you have been primarily responsible for during the reporting period. This report must be submitted on the anniversary of the start of your training program and on application to take the certifying exam.
The ACVAA is in the process of transitioning resident case logs from an excel file to an online format. At this time, the work on this new online format is still in progress so please do not use the online form. Until you read otherwise, the excel file will be used for case logs. The excel log file and the sample log file are listed under “Documents and Forms. ” Watch this space for updated information.
